Athletic Clearance
All students interested in participating in any MHS sport, must complete the Athletic Clearance process, to include an up-to-date physical on file. Find the process and forms below.
The student's athletic clearance must be approved prior to team work outs and try outs.
Physical Clearance Procedure:
Go to AthleticClearance.com (CLICK HERE) and create a player account for 2025-2026 school year.
Complete EL2 form (CLICK HERE) using updated 2025 form
Make sure page 4 of the EL2 (physical) is signed and dated by the Physician, Parent and Student. This is the ONLY page of the EL2 you need to scan and upload.
Complete the Form D - Off-Campus form 2025-2026
Complete the Form E - Parent Permission for Student Driver/Rider/Transport
Complete the Santa Rosa Pre-Participation Physical (Insurance) Form 2025-2026 (CLICK HERE) and have it notarized
If you have moved into the school district and will be attending Milton High School for the first time or live outside the school district complete a GA4 form (CLICK HERE)
You will receive the confirmation page once completed. Failure to complete all steps will delay your son or daughter from Participating in Athletics at Milton High School.
